MyHealthCheckup.com is a health management Web site trusted by its clients and partners. All users' information is maintained in the strictest of confidence within the bounds of the law by MyHealthCheckup. A user's access to and use of the Web site or other related services constitutes information voluntarily given by the user. Once you are connected to MyHealthCheckup, all entrusted information is stored on our secure servers.
This information is only saved on our server if you choose to register and establish a personalized health file with a unique identity and password.
During registration we will ask for your email address so we can communicate with you (e.g. send you content updates) and also to register you on our site. Date of birth, gender, and other health information is asked to provide you with a highly personalized cardiometabolic risk profile.
MyHealthCheckup does two main things with your information:
First, we provide you with personalized cardiometabolic profiles and lifestyle treatment programs to help address specific risk factors. For example, your cardiometabolic risk profile identifies your health risks and our lifestyle management tool recommends steps that you can take to prevent medical problems in the future or to enhance your health. Your health file is accessible only by you unless you decide to give access to your physician or other health care professionals.
Second, we use the information to perform statistical analyses and to profile user behaviors and characteristics. These statistical analyses help us measure interest in and use of the various areas of the site. Your data is combined with that of other users to produce an overall population profile. These statistics may be used to determine health and wellness needs, identify opportunities for future health and wellness programs, and measure program effectiveness. It also allows us to provide aggregate information (information about a group of people but containing no personally identifiable data) on the health status of a population to our sponsors and the research community. No personally identifiable information is ever disclosed.
MyHealthCheckup stores the Internet Protocol (IP) address of your computer when you visit the Web site. This information is only used to create broad demographic summaries of where our users come from: for example, we know what percentage of users come from Europe or South America, but our use of the IP addresses does not include identifying who those users actually are.
MyHealthCheckup has built a robust security wall around personal information. The technologies we use for Privacy and Confidentiality offer numerous layers of protection around the information. As we are dedicated to enforce our Privacy Policies at all times we also incorporate new technologies when new security is needed.
Each user has a unique ID to enter the Web site. This ID along with a password enables proper identification of users regardless of their entry to the Web site. Upon entry, MyHealthCheckup ensures that users connect securely to their own personal health information. When exiting the Web site (by browsing to another address for example), the user cannot go back and access his/her personal information. The ID and password must be re-entered. Inactivity of a record for a certain period of time also logs out the user automatically.
MyHealthCheckup may employ cookie technology to allow users to personalize the use of the Web site. Cookies are small files a Web site can use to recognize repeat users and facilitate the user's ongoing access to and use of the site. Generally, cookies work by assigning a unique number to the user that has no meaning outside the assigning site. They do not pose a threat to a user's files. We currently use cookies as a timer that helps increase the security around your record by turning off access after a certain period of inactivity. We may consider the use of cookies for other purposes in the future as technology meets our privacy standards. However, if you do not wish to receive cookies, or want to be notified of when they are placed, your browser may permit you to do so.
MyHealthCheckup utilizes state-of-the-art firewall technology to ensure the highest level of protection for Personal Information.
MyHealthCheckup believes that individuals have the right to keep their personal health information confidential and that the health information you entrust us to hold and maintain belongs only to you. No one has the right to access or review your information without your authorization.
MyHealthCheckup cannot guarantee the reliability of hardware, software and networks outside of MyHealthCheckup's physical facilities. Neither MyHealthCheckup, nor its clients or partners, assumes responsibility for any misconnection to and/or failed transmission resulting from the use of any network or facility provided by the Internet and/or telecommunication networks. MyHealthCheckup will not be held responsible for any damages, including but not limited to, loss of data, delays, non- or miss-delivery, or service interruptions.
You expressly agree that exclusive jurisdiction for any dispute with MyHealthCheckup, or in any way relating to your membership or use of the MyHealthCheckup Web site, resides in the courts of Quebec, Canada and you further agree and expressly consent to the exercise of personal jurisdiction in the courts of Quebec in connection with any such dispute including any claim involving MyHealthCheckup or its affiliates, subsidiaries, employees, contractors, officers, directors, telecommunication providers and content providers. This Privacy Statement is governed by the laws of Quebec without respect to its conflict of laws principles. If any provision of this Privacy Statement is found to be invalid by any court having competent jurisdiction, the invalidity of such provision shall not affect the validity of the remaining provisions of this Privacy Statement, which shall remain in full force and effect. No waiver of any of this Privacy Statement shall be deemed a further or continuing waiver of such a statement.
